Job summary

UC Irvine Health seeks a skilled Case Manager to lead high-level assessment and care planning for a defined patient group. You will collaborate with an interdepartmental team to ensure cost-effective, high-quality discharge planning and patient care.

Must hold an active nursing, social work, or licensed therapist license and have at least 5 years of clinical experience with case management; BSN is required for external candidates. Excellent communication and computer skills are essential.

Qualifications

• Minimum 5 years of clinical experience and 1 year of case management experience or as a UCI employee meeting clinical requirements and has a BSN
• Current Licensed Registered Nurse, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist allows for independent assessments
• Excellent written and verbal communication skills in English

Responsibilities

• Provide high-level assessment and care planning for identified patient groups
• Coordinate with interdepartmental teams to ensure high-quality, cost-effective discharge planning
• Lead interdisciplinary rounds and maintain effective working relationships across the Health System
