Job summary

UCI Health is seeking a dedicated Case Manager with a Bachelor of Science in Nursing and a current California RN license to assess patient needs, develop and monitor care plans, and coordinate interdisciplinary care. The role emphasizes safe, timely, and patient-centered outcomes in a busy academic health system.

Preferred candidates have 3–5 years in acute care and 1–2 years in case management, with strong typing, communication, and MS Office skills to support documentation and collaboration

Qualifications

• BSN required for external candidates
• Current California RN license
• American Heart Association BLS Certification
• Three to five years of experience in an acute care hospital setting (medical/surgical preferred)
• One to two years of experience as a Case Manager
• Must be proficient in typing and in writing/communicating professionally
• Proficient in computer skills including Microsoft Office
• Ability to establish and maintain effective working relationships across the Health System
• Excellent written and verbal communication skills in English

Responsibilities

• Assess treatment needs and develop, monitor, and evaluate treatment plans and progress
• Coordinate and provide care that is safe, timely, effective, efficient, and patient-centered
• Facilitate interdisciplinary approaches to patient care
• Collaborate with healthcare teams to ensure continuity of care and appropriate resource utilization
