- Aid in implementing clinical and individual programs
- Assist with daily implementation of DHSP, activity schedules, and recommended plans
- Provide support to persons served in a day habilitation setting
- Provide instruction and skill building in self-help, sensorimotor, communication, social, independent living, behavioral, affective, and wellness development
- Assist with case management responsibilities
- Provide input for interdisciplinary team reviews
- Assist in developing DHSPs and maintain contact with families, funding sources, and staff
- Implement clinical and daily plans and recommend or implement program improvements
- Maintain daily documentation, collect data, and complete progress notes
- Work individually and in groups to develop skills
- Keep members engaged in meaningful activities and offer choices
- Communicate with management regarding treatment and programs
- Participate in case management and other teams
- Support members in crisis management using Positive Behavior Support and Safety Care
- Assist with staff training
- Maintain program-area upkeep and monitor, clean, and store supplies and equipment
- Advocate for persons served and support community integration through community, volunteer, reverse-integration, and friendship opportunities
- Perform other related duties as required
Requirements
- One year of relevant work experience/training
- A high school diploma or equivalent is required
- Valid driver's license presented at employment
- At least 21 years of age
- Maintained a valid US driver's license for at least six months
- Pass a driver's screening background check
- Adequate insurance and safe driving record if using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with daily living tasks
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Close, distance, color, peripheral, and depth vision; ability to adjust focus
- CPR within 2 weeks of hire
- First Aid within 2 weeks of hire
- New Employee Training (NET)
- Safety Care within 90 days of hire
- Knowledge of advocacy techniques, applied behavioral analysis with developmental disabilities populations, communication styles, empowerment and self-advocacy techniques, formal and informal assessment practices, participatory planning techniques, physical personal management skills, independent living management skills, and crisis intervention techniques
