- Provide individual and group support to people served in a residential program

- Perform hands-on program tasks and provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds and maintain records, progress notes, data, communication logs, and activity schedules
- Procure, administer, document, and dispose of medication consistent with MAP requirements and applicable policies
- Communicate with nurses, case managers, Visiting Nurses Association, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with regulatory, corporate, and divisional policies and procedures
- Perform housekeeping, snow shoveling, trash removal, and home safety and maintenance
- Provide shift coverage in other homes or programs as assigned
- Act as a human rights advocate and provide physical intervention or First Aid during emergencies
- Assist with developing, implementing, and documenting ISP/PSTPs and clinical programs
Requirements
- One or more years of experience and/or training in a related function encouraged
- High school diploma or equivalent required; in some cases, experience may be substituted for academic training
- Valid driver's license required at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record if using a personal vehicle
- Ability to pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with activities of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, communication styles, household management, physical personal management, and Positive Behavior Support treatment programs
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
