Job description

  • Provide individual and group support to people served in a residential program
  • Teach skills that enhance independence, quality of life, and self-advocacy
  • Maintain a safe, secure, and clean residential environment
  • Help individuals integrate into their communities and develop and maintain friendships
  • Assist with community-based supports, resources, recreational activities, social inclusion, and community engagement
  • Support choices, decision-making, family partnerships, employment, volunteering, and independent living
  • Provide training in behavioral and rehabilitative interventions, fire and safety, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
  • Support access to assistive technology and adaptive equipment
  • Perform hands-on program tasks and provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds and maintain records, progress notes, data, communication logs, and activity schedules
  • Procure, administer, document, and dispose of medication consistent with MAP requirements and applicable policies
  • Communicate with nurses, case managers, Visiting Nurses Association, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure compliance with regulatory, corporate, and divisional policies and procedures
  • Perform housekeeping, snow shoveling, trash removal, and home safety and maintenance
  • Provide shift coverage in other homes or programs as assigned
  • Act as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Assist with developing, implementing, and documenting ISP/PSTPs and clinical programs
Requirements
  • One or more years of experience and/or training in a related function encouraged
  • High school diploma or equivalent required; in some cases, experience may be substituted for academic training
  • Valid driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Adequate insurance and safe driving record if using a personal vehicle
  • Ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with activities of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • PBS Tier 1 required within 150 days of hire
  • Safety Care required within 90 days of hire
  • Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, communication styles, household management, physical personal management, and Positive Behavior Support treatment programs
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to communicate effectively in oral and written formats
