Job summary

Vinfen is seeking a dedicated professional to implement clinical programs, develop individual plans, and support community integration for persons served. You will work with an interdisciplinary team to ensure crisis intervention and skill development objectives are met.

Qualifications

  • One year of relevant work experience or training.
  • High school diploma or equivalent.

Responsibilities

  • Implement clinical and individual programs, daily schedules, activities, and plans.
  • Provide instruction in self-help, sensorimotor, communication, social, independent living, behavior, affective, and wellness development.
  • Assist with case management responsibilities and provide input for interdisciplinary team reviews.
  • Assist in developing DHSPs and maintain contact with families, funding sources, and staff.
  • Implement clinical and daily plans and recommend or implement program changes.
  • Maintain daily documentation, data collection, and progress notes.
  • Work individually and in groups to develop skills and keep members engaged in meaningful activities.
  • Communicate with management regarding treatment and programs.
  • Participate in case management and other interdisciplinary teams.
  • Support members in crisis management using Positive Behavior Support and Safety Care.
  • Assist with staff training as needed.
  • Maintain program-area upkeep and monitor, clean, and store supplies and equipment.
  • Advocate for persons served and support community integration through community, volunteer, reverse-integration, and friendship opportunities.
  • Perform other related duties as required.

  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist clients with daily living tasks
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to communicate with clients, staff, and external representatives
  • CPR within 2 weeks of hire
  • First Aid within 2 weeks of hire
  • New Employee Training (NET)
  • Safety Care within 90 days of hire
