Job summary

Paycom - ATS seeks a Crisis and Respite Case Manager (QMHP-CS) to provide ongoing case management and rehabilitation services to adults with mental illness in community and respite settings. The role requires coordinating recovery plans, communicating with clients and families, and collaborating with ACT team members.

Extensive travel within the region is required. Responsibilities include maintaining electronic health records, tracking census and expenses, and participating in weekly staff

Qualifications

• Bachelor’s Degree in a human services field.
• QMHP-CS credentialing required.
• At least 1 year experience working with adults with serious and persistent mental illness.
• Texas driver’s license with good driving record.
• Ability to work independently and as part of a team.
• Bilingual ability (Spanish/English) preferred.
• Must be able to travel extensively within the region.

Responsibilities

• Provide ongoing case management and rehabilitation services for adults with mental illness.
• Coordinate recovery plans and services across office, home, respite facility, and community locations.
• Maintain electronic health records and document across processes.
• Travel between locations as assigned.
• Attend weekly staff meetings and participate in data collection activities.
