Qualifications

  • Bachelor’s Degree in a human services field.
  • QMHP-CS credentialing required.
  • At least 1 year experience working with adults with serious and persistent mental illness.
  • Texas driver’s license with good driving record.
  • Ability to work independently and as part of a team.
  • Bilingual ability (Spanish/English) preferred.
  • Must be able to travel extensively within the region.

Responsibilities

Job description

DescriptionUnder the supervision of the Clinic Manager, the Crisis and Respite CM provides ongoing case management services and rehabilitation services for a small caseload of adults with mental illness. Individuals may be residing in the community or temporarily admitted into our crisis respite facility. Coordinates and provides services. Must be able to make effective assessments and interventions in situations involving mental health services provided to individuals. Ability to network for community agencies, probation/parole, law enforcement, and other social service agencies to facilitate services for adults. Responsible for coordination, planning, and implementation of the recovery plan. Ability to communicate with adults and their families to promote collaboration when creating recovery plan. Must be a self-motivator, able to work independently and with ACT team members, and remain flexible to solve problems and crises. Extensive in-region travel is required to provide services. Out of region travel may be required occasionally for trainings. Services may be provided at the office, in-home, crisis respite facility, or other community locations as needed to meet with needs of those served. Voluntary on-call during evening and weekend hours available. Must be familiar with, and/or willing to be trained in, trauma-informed care. This includes understanding, recognizing, and responding to the effects of all types of trauma, and an ability to work in a behavioral environment where staff convey dignity, respect, hopefulness, and the opportunity for choice and empowerment among consumers.The Respite CM will assist the Respite Program Manager in activities at our crisis respite facility. When performing crisis respite activities, the Respite Program Manager will be the supervisor. Activities could include managing the daily census spreadsheet, tracking grocery expenses, admit and discharge individuals from respite beds in our electronic health record. Direct services at respite will be provided individually and in groups.Essential FunctionsProvide ongoing case management services and rehabilitation services for adults with mental illness to assigned caseload as determined by Supervisor or Program Manager. Typically, admissions to the respite facility are short-term, and the ability to be flexible and quickly connect individuals to the needed resources will be required. Knowledge of adult issues in a rural setting. Acts as a positive role model to those served and advocates on their and their families’ behalves. Duties include but are not limited to the following:Maintain understanding of TRR services and UM Guidelines for all LOCs.Accurately complete a uniform assessment to identify an individual’s needs and strengths as appropriateMust be able to utilize an electronic health record system to document collaboratively and concurrently with consumers across all processes, including assessment, service planning, and ongoing consumer-provider interactionMeet productivity standards and other performance standards set by the agencyProvide community referrals as neededCommunicate with referring entities, internal and external providersMaintain a flexible work schedule while demonstrating effective time management to meet needs of individuals and families servedTravel between locations as assignedParticipate in weekly staff meetingsParticipate in data collection activities and peer review processes as requestedOther duties as assigned by supervisorSkills and QualificationsA successful candidate will have the following abilities and minimum qualifications:Bachelor’s Degree in a human services field and be able to obtain credentialing as a QMHP-CS is requiredPrefer at least 1 year experience working with adults with serious and persistent mental illness in the community or residential settingWorking knowledge of mental health and substance abuse services and needsClear understanding of normal and atypical developmentMust have strong computer operating skillsAbility to participate respectively as a team member during department and agency meetingsExcellent verbal and written communication skillsAbility to work both independently and part of a team to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard practicesCapacity for flexibility in dealing with ongoing changes in the service delivery systemBilingual ability (Spanish/English) is preferred but not requiredMust have a valid Texas driver’s license, a good driving record for the past three years, and personal automobile liability insurance as required by the state of TexasMust be able to bend, stoop, kneel, crouch, squat, lift at least 25 pounds, reach overhead, and sit for extended periods of time. Must be able to walk over a variety of surfaces.
