Job summary

Paycom - ATS is seeking a dedicated Case Manager for the Residential program to support adults with serious psychiatric disorders and co-occurring conditions. You will coordinate admissions, arrange transportation, and help develop discharge plans to promote successful community reintegration.

This role requires an associate or bachelor’s degree in social work, psychology, or related field, at least two years in case management, and strong communication and problem-solving skills.

Qualifications

• Associate or bachelor’s degree in social work, psychology, counseling, or related field.
• Minimum of 2 years of experience in case management or related social services.
• Experience with individuals with serious and persistent psychiatric disorders, substance use disorders, or involvement with the criminal justice system is a plus.
• Strong communication, organizational, and problem-solving skills.

Responsibilities

• Provide case management services in the client’s environment of choice with a focus on daily living, housing, income, health, and community resources.
• Coordinate admissions and arrange transportation to support services and housing resources.
• Develop discharge plans to support successful community reintegration.
• Record progress and adjustments in individualized charts and coordinate with other service providers.
• Advocate for clients to receive required services and maintain professional relationships.
