Job summary

Apis Services, Inc. is seeking a Case Manager to assess participant needs, coordinate service plans, and advocate for access to housing, health care, employment, and social supports. You will work with participants, families, and providers to help achieve recovery goals and improve quality of life.

The role requires collaboration across teams, documentation accuracy, and adherence to HIPAA and agency policies. Community-based and office-based duties are included, with local travel as needed.

Qualifications

• Bachelor's Degree in Human Services, Social Work, Psychology, Counseling, Sociology, Behavioral Health, or related field required.
• 1+ year of case management experience preferred.
• Valid driver’s license with acceptable driving record.

Responsibilities

• Conducts comprehensive assessments to identify participant strengths, needs, preferences, and barriers to success.
• Develops, implements, and monitors individualized service and treatment plans.
• Advocates for participants to ensure access to services, supports, and community resources.
• Provides community-based case management and support services.
• Documents time worked accurately and maintains compliant documentation.
