Job summary

You will assess, triage, and refer participants for appropriate follow-up treatment to restore an effective lifestyle and may provide outreach to outpatient clients as needed. Qualifications

• Must have crisis intervention training and ability to provide services to participants and community.
• Experience in counseling or case management for SMI participants in Mental Health programs.
• Bilingual English/Spanish preferred to serve diverse community.

Responsibilities

• Perform face-to-face crisis assessment and immediate intervention.
• Assess, evaluate, triage, and refer participants for follow-up treatment.
• Engage in outreach to outpatient clients as needed and support treatment planning.
