Job summary

Horizon House is actively seeking a D & A Professional to provide crisis intervention under clinical supervision within the PACT multidisciplinary team. This role involves engaging with adults with complex behavioral health needs and contributing to rehabilitation and support services.

The position focuses on conducting assessments, delivering treatment, educating families, and coordinating care across office and community settings, with emphasis on client-centered goals and safety.

Qualifications

• Bachelor’s Degree in a Behavioral Health-Related field and has worked with adults with severe and persistent mental illness or similar human-services needs.
• CAC CADC certification required, with ongoing coursework to qualify for re-certification.
• Knowledge of multiple support systems for substance abusers.
• Skills and competence to establish trusting relationships with persons with severe mental illnesses and respect for clients’ rights.
• Good communication skills.
• Ability to work independently and as part of a focused team.
• Must have a valid driver’s license with an acceptable driving record.

Responsibilities

• Provide crisis intervention under clinical supervision of staff with professional degrees.
• Carry out rehabilitation and support functions as part of the PACT team.
• Assist in treatment, provide substance abuse services, education, support and consultation to families.
• Participate in client-centered comprehensive assessments of psychiatric history, mental status, and diagnosis.
• Document client progress to maintain a permanent record; assist in individual and group treatment in various settings.
• Assess and adjust treatment plans in response to changes in mental status or risk.

Education

Bachelor’s Degree in Behavioral Health-Related Field

Certified Addiction Counselor (CAC) / CADC required

Good communication skills

Ability to work independently and as part of a team

Valid driver’s license with acceptable driving record
