Job summary

Lakeview Center is seeking a Mobile Response Team case manager to support adults with serious mental illness by coordinating access to medical, social, and behavioral health services. You’ll deliver in-home and community-based care, work with a multidisciplinary team, and help clients achieve self-sufficiency in daily living, learning, work, and social activities.

Travel locally to clients’ homes and training sites is required; strong communication, computer skills, and problem-solving abilities

Qualifications

• Bachelor’s degree in a related human services field is required.
• 1 year of full-time or equivalent experience with adults experiencing serious mental illness is required or.
• Bachelor’s degree with 3 years full-time or equivalent experience with adults with serious mental illness is acceptable.

Responsibilities

• Provide in-home and community mental health case management services.
• Travel locally to clients’ homes, training locations, and other sites as needed.
• Coordinate access to medical, social, educational, and other services for clients.
• Maintain client records and complete required trainings and compliance tasks.
