Telecare Corporation is seeking a Case Manager for its 24/7 Mobile Crisis Response Team (MCRT) in Stockton, CA. You will collaborate with clinicians to respond to mental health or substance use crises in the community, delivering short-term, field-based care aimed at stabilization and connection to ongoing treatment.

Responsibilities include de-escalation, risk assessment in the field, and potential call screening for the mobile team.

Qualifications

• Master's degree in Social Sciences is required.
• One year of experience in mental health services.
• Valid and current driver's license with personal vehicle insurance.

Responsibilities

• Respond to behavioral health crisis with a clinician partner in community settings.
• Provide immediate, field-based, short-term intensive services to individuals in crisis.
• Stabilize and de-escalate individuals and connect them to ongoing care.
• Help divert from unnecessary hospitalization or involvement with the criminal justice system.
• May be assigned to take calls and screen calls for a mobile team's response.
