Lakeview Center Inc. in Florida is seeking a Case Management professional to provide in-home and community services for adults with serious mental illness.

The role emphasizes coordinating access to medical, social, and educational resources while working with a multidisciplinary team. You will travel locally to clients’ homes and training sites, complete required case management training within three months of starting, and maintain up-to-date knowledge of community resources and state

Formación

• Bachelor's degree in a related human services field is required.
• Minimum 1 year of full-time or equivalent experience with adults experiencing serious mental illness.
• Alternatively, 3 years of equivalent experience with adults with serious mental illness.
• Must be able to travel locally to clients' homes and training locations.
• Ability to complete approved mental health targeted case management training within 3 months.

Responsabilidades

• Provide in-home and community case management services.
