Job summary

The Kids for the Future is seeking a dedicated Crisis Care Navigator to support clients within our Tulsa, OK stabilization services. You will coordinate care, monitor documentation, and serve as the primary liaison between the crisis unit and partners to ensure timely transitions.

Responsibilities include maintaining HIPAA-compliant records, collaborating with multidisciplinary teams, and assisting with insurance pathways while using Eleos for eligible services.

Qualifications

• High School Diploma or equivalent required.
• ODMHSAS Case Manager certification required.
• Current and valid Driver’s License required.
• Reliable transportation and ability to travel up to 25%.

Responsibilities

• Maintain standardized communication processes on the unit.
• Act as liaison between the crisis unit and care coordination partners.
• Oversee documentation for client transitions between levels of care.
• Identify local referral sources to meet client needs.
• Ensure Release of Information and chart documentation are complete.
• Complete assessments to identify and prioritize client resources.
