Job summary

Rimrock Foundation is seeking a Client Navigator to provide care coordination and linkage across medical, behavioral health and community services. You will conduct screenings, facilitate referrals, and support crisis response to ensure timely access to care.

This role emphasizes trauma-informed, culturally responsive, person-centered approaches with required documentation and collaboration in an integrated care setting.

Qualifications

• Bachelor's degree in human service or social work required or equivalent experience.
• One year of experience in mental health or SUD field preferred.

Responsibilities

• Conduct initial intake and screenings per CCBHC standards and identify urgent needs.
• Coordinate timely access to services and facilitate referrals to internal and external providers.
• Respond to crisis needs and coordinate 24/7 crisis service connections.
• Participate in interdisciplinary teams and document care coordination activities.
• Assist clients in identifying goals and link to resources across care levels.
• Maintain accurate timely documentation per CARF/HIPAA and state rules.
• Communicate effectively with clients, families, and partners while protecting privacy.
• Apply CCBHC model knowledge to integrated care and whole-person approaches.
• Engage in ongoing training and complete at least 10 hours yearly.
• Perform additional duties as assigned.
