Job summary

MultiCare’s Greater Lakes Mental Health division seeks a Crisis Care Coordinator to provide follow-up services for individuals in crisis and coordinate referrals to community resources to stabilize and mitigate future crises.

The role involves risk assessment, care coordination across systems, and collaboration with law enforcement and co-responders to divert from incarceration or unnecessary ER visits.

Qualifications

• Bachelor’s degree in social sciences, mental health or related field is required.
• Master’s degree preferred.
• ACCR credential must be applied for within 30 days; LMHCA credential may substitute.

Responsibilities

• Provide follow up services for individuals seen in crisis.
• Coordinate referrals and warm hand-offs to appropriate community services.
• Conduct risk assessments and ensure linkage to services.
• Collaborate with law enforcement and other first responders to educate.

Education

Bachelor’s degree in social sciences, mental health or related field
