Job summary

Ascension is seeking a dedicated care coordinator in Tulsa, OK to guide Medicare patients through post-discharge plans and connect them with follow-up services. You will perform mental health screenings (PHQ-9, GAD-7) and risk assessments, and help patients navigate referrals to therapists and community resources.

Qualifications

• BLS Provider preferred (American Heart Association or American Red Cross accepted).
• CCMC Case Manager credential preferred.
• Counselor or Social Worker background preferred.

Responsibilities

• Guide Medicare patients through post-discharge care plans and ensure follow-up services are utilized.
• Conduct mental health screenings (PHQ-9, GAD-7) and risk assessments.
• Bridge patients to mental health resources and therapeutic services.
• Direct patients to appropriate interventionalists and care pathways for timely, targeted care.
