Job summary

Unison Health is seeking a Crisis Care Navigator to support individuals experiencing behavioral health crises across outpatient and hospital ED settings. The role focuses on rapid engagement, care coordination, and linking clients to appropriate services.

The position requires a related degree, CPI/CPR certifications after hire, and flexibility to work evenings, weekends, and holidays. You will collaborate with teams to ensure seamless transitions and follow-up within a week.

Qualifications

• Two- or four-year degree in social work, psychology, or related field.
• High school diploma required.
• CPI and CPR certifications required after hire.

Responsibilities

• Provide care navigation and support for behavioral health clients in the Emergency Department who do not require hospitalization.
• Coordinate follow-up care within 7 days of ED visit.
• Identify barriers to treatment engagement and coordinate resources.
• Arrange transportation and connect clients with community services.
• Assist with insurance and service enrollment.
• Maintain documentation and program data as required.
• Collaborate with providers and community partners to ensure continuity of care.
• Facilitate warm handoffs and monitor engagement until transition is complete.
• Participate in supervision, meetings, trainings, and professional development.
• Adhere to agency policies, procedures, and performance expectations.

Education
