Job summary

MultiCare Health System is seeking a Crisis Care Coordinator to provide follow up services for individuals seen in crisis and to coordinate referrals to community resources. The role includes conducting risk assessments and ensuring linkage to appropriate services to stabilize crises and avert unnecessary ER visits.

You will collaborate with the crisis team and law enforcement to educate and support care transitions, aiming to divert individuals from incarceration and promote community safety

Qualifications

• Bachelor’s degree in social sciences, mental health or related field required.
• Master’s degree preferred.
• ACCR credential must be applied for within 30 days of hire.
• LMHC/LMHCA credential substitution allowed.
• Current Washington state driver’s license.

Responsibilities

• Provide follow up services for individuals seen in crisis and coordinate referrals.
• Assist in warm handoffs to community services to stabilize or mitigate crisis.
• Conduct risk assessments and coordinate care across systems.
• Collaborate with law enforcement and other first responders to educate and support the crisis team.
