CHR is seeking a Credible Messenger for the Hartford, CT area to engage high-risk youth and families in the Credible Messenger Program. The role focuses on intake, case coordination, and connecting youth with community supports outside traditional office settings.

Candidates should bring lived experience in the Hartford community, strong rapport-building skills, and a passion for advocacy and non-traditional hours including evenings, with a commitment to safe, in-home and community-based

Competenze

• Lived experience within Hartford CT communities preferred.
• Experience building trust with high-risk youth and families.
• Knowledge of local resources for youth in Hartford.

Mansioni

• Coordinate intake, engagement, and ongoing services for referred youth.
• Serve as liaison between CHR, CSSD/JBCSSD, clients and families to support communication and planning.
• Link clients to behavioral health, housing, education, employment, and other services.
• Assist with scheduling, transportation, and other supports to improve engagement.
• Maintain accurate documentation in accordance with program standards.
• Participate in supervision, meetings, trainings, and other duties.
