Community Health Resources (CHR) in Hartford, CT is seeking a Credible Messenger for the Credible Messenger Program. This is a full-time, 40-hour/week position with a Monday-Friday schedule and some evenings.

The role focuses on youths with lived Hartford experience, building trust, coordinating intake, and linking families to community supports. A valid driver’s license and strong community knowledge are required.

Qualifikationen

• Have lived experience in Hartford communities and familiarity with Hartford neighborhoods.
• Ability to build trust and rapport with high-risk youth and families.
• Knowledge of local resources available to youth in Hartford.
• Experience collaborating with Juvenile Probation Officers or the court system.
• Enjoy working in community and in clients’ homes, not office-based.
• Flexible schedule including some evenings and outside traditional hours.
• Motivated to support and advocate for disadvantaged youth.
• Creative and persistent engagement strategies.

Aufgaben

• Coordinate intake, engagement, and ongoing services for youth referred to program requirements.
• Liaise between CHR, CSSD/JBCSSD, families, and community providers.
• Link clients and families to behavioral health, housing, education, and other supports.
• Assist with appointment scheduling, transportation, and practical supports.
• Maintain timely documentation of services and referrals.
• Participate in supervision, meetings, trainings, and other duties.
