Community Health Resources seeks a Credible Messenger in Hartford, CT. You will engage high-risk youth and families, coordinate services, and connect them with community supports to reduce re-arrests and promote prosocial outcomes.

This role emphasizes lived experience, local knowledge, and collaboration with CSSD/JBCSSD. Flex hours, some evenings, and a driver’s license are required.

Qualifications

• Live in the communities where the youth will be served.
• Have transformed personal experiences into positive outcomes for youth.
• Must have a valid driver’s license.
• Attend all required CHR/CSS training programs.

Responsabilités

• Coordinate intake, engagement, and ongoing services for youth referred by CSSD/JBCSSD per contract and ISP.
• Serve as liaison among CHR, CSSD/JBCSSD, clients, families, supports, and providers.
• Link clients to behavioral health, housing, benefits, education, and employment resources.
• Assist with appointment scheduling, transportation, and other supports to promote engagement.
• Maintain timely documentation of services and communications per standards.
• Participate in supervision, meetings, trainings, and other duties as assigned.
• Perform other duties as required.
