Southwest Behavioral & Health Services is seeking a Community Outreach Navigator - Court Liaison to join our team in Phoenix. The role focuses on court-based navigation for justice-involved homeless individuals, coordinating with city and court partners to implement treatment alternatives to incarceration while providing direct behavioral health services.

The position requires a 21+ age minimum, fingerprint clearance, and a valid Arizona driver's license.

Qualifikationen

• Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight weeks.
• Must maintain a valid Arizona driver’s license and pass motor vehicle screening for insurance purposes to transport clients.
• Must maintain personal auto insurance per SBH guidelines.
• Must pass the Maricopa County clearance to enter the court and jails and level 1 fingerprint clearance.
• Minimum age of 21 years.
• High School Diploma or GED, some college plus at least one year directly related job experience.

Aufgaben

• Works in a Navigation Role providing Court Based Navigation Services for Justice-involved homeless individuals.
• Collaborates with City of Phoenix, Police, Prosecutors, Court System, Judges, and Program Liaisons to achieve contract goals.
• Provides Direct Care; Assessment, Diagnosis, and Case Based Treatment Planning for persons served.
• Performs screenings and eligibility assessments and facilitates early identification/qualification processes.
• Enrolls individuals in benefits to support services.
• Develops case plans with Prosecutors/Judges for alternatives to incarceration.
• Arrives in court prepared with a working case plan.
• Provides crisis intervention, de-escalation, and Behavioral Health interventions.
• Assesses for and facilitates Integrated Care-based referrals and essential healthcare, medications, equipment.
• Conducts jail visits to prepare persons for court as needed.
• Advocates in court for persons served.
