Job summary

Open Sky Community Services is seeking a Community Services Navigator to provide case management and direct outreach for individuals in community services programs. The role involves developing Support Plans, coordinating with agencies, and ensuring access to transportation for medical and behavioral health needs.

The position requires a high school diploma, a valid driver's license, and at least 2 years of experience with homelessness, justice involvement, or mental health/substance use

Qualifications

• Excellent communication, documentation, time management and organizational skills required.

Responsibilities

• Develop a Support Plan with the member that guides services and aligns with other care plans.
• Complete Assessments and Support Plans within prescribed timelines and specifications.
• Act as liaison between members, community agencies and families to coordinate treatment, employment and education supports.
• Coordinate with medical and behavioral health providers to obtain active care plans.
• Provide temporary transportation assistance to essential medical/behavioral health appointments.
• Participate in On-Call rotation.
