Job summary

Open Sky Community Services is seeking a Community Supports Navigator to provide case management and direct outreach to individuals at high risk for admission to 24-hour treatment settings. You will develop personalized Support Plans, complete assessments on schedule, and coordinate with medical, behavioral health, and community partners to ensure continuous care.

The role requires a valid driver’s license and at least 2 years of experience working with homeless or justice-involved individuals

Qualifications

• Requires strong communication, documentation, time management and organization.
• High school diploma or GED required.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Develop a Support Plan with the member that guides services and aligns with existing care plans.
• Complete all Assessments and Support Plans within prescribed timelines and specs.
• Act as a liaison between members, families, and community agencies to coordinate treatment.
• Collaborate with medical and behavioral health providers to obtain active care plans.
• Provide temporary transportation assistance to medical/behavioral health appointments.
• Participate in an On-Call rotation.
