Responsibilities

• Performs mental health screening via standardized intake and follow-up screenings.
• Acts as case coordinator linking patients to facility-based programming, psychiatric services, and community resources.
• Conducts wellness checks and supportive engagement to monitor patient well-being.
• Provides crisis intervention in mental health emergencies.
• Maintains timely, accurate documentation in the electronic health record and ensures compliance with jail standards.
• Collaborates with psychiatry providers, nurses, custody staff, and program coordinators to support mental health service delivery.
• Performs other duties as assigned by supervisor.

Major/Essential Functions

• Mental Health Screening: Conducts standardized intake andfollow-up screenings.
• Case Management and Coordination of Care: Serves as a casecoordinator for assigned patients by linking them to facility-basedprogramming, psychiatric services, and reentry or communityresources as appropriate.
• Wellness checks and supportive Engagement: Performs routinecell-side activities to motor patient well-being.
• Crisis Intervention: Responds promptly and effectively tomental health crisis.
• Documentation and Compliance: Maintains timely and accuratedocumentation in the electronic health record and ensurescompliance with jail standards.
• Team Collaboration: Works collaboratively with psychiatryproviders, nurses, custody staff, and program coordinators tosupport the delivery of mental health services.
• Other duties as Assigned: Willingly performs all otherassignments as directed by the superior.
