Job summary

Flagstar Rehab is seeking licensed Occupational Therapy Assistants (OTA) to provide skilled nursing facility therapy services in New York. The role offers flexible scheduling with full-time, part-time, or per diem shifts and competitive hourly pay of $34–$40.

On-site SNF settings require active NYS OTA license, BLS, and EMR proficiency; REHAB OPTIMA or Point Click Care experience is a plus. Weekly pay, benefits options, and supportive onboarding help you grow while delivering patient care.

Qualifications

• EMR competency required for patient charting and progress notes.
• Current NYS OTA License and Registration.
• Valid BLS Certification is required.
• Meets all State and Federal employment requirements for US in a Nursing Facility.

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to SNF patients with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, cancer, orthopedic surgeries.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Requirements and Responsibilities:

• Current NYS OTA License and Registration
• Valid BLS Certification
• EMR Competence (Proficiency in REHAB OPTIMA + POINT CLICK CARE a plus!)
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
