Job summary

Flagstar Rehab is seeking Licensed Occupational Therapists to join skilled nursing facilities. You will perform evaluations, develop treatment plans, and supervise COTAs in a SNF setting.

Eligible candidates hold a NYS OT license with registration, BLS, and EMR proficiency. The role offers Full-Time/Part-Time/Per Diem schedules with weekly pay options and benefits, including paid orientation and commuter benefits.

Qualifications

• Current NYS OT License and Registration.
• Valid BLS Certification.
• EMR proficiency.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to SNF residents with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery and related diagnoses.
• Monitor and document patient progress using EMR software.

Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Occupational Therapist Requirements and Responsibilities:

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
