Flagstar Rehab is seeking a Certified Occupational Therapy Assistant (COTA) to join our skilled nursing facility team in New York. You will work closely with the OT Supervisor to follow the Plan of Care, deliver high-quality therapy, and help patients improve function and cognition.

Qualifications

• Current NYS OTA License and Registration.
• EMR Competence.
• State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation).
• Valid Proof of Identity and Work Authorization.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress with the use of EMR software.
• Proficiency in Rehab Optima + Point Click a plus.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Job Duties:
