Job summary

Flagstar Rehab is seeking a Certified Occupational Therapy Assistant (COTA) to join our Skilled Nursing Facility team. You will work closely with the OT Supervisor, delivering quality therapy and educating patients and families to improve daily functioning and cognition.

We offer direct employment or Independent Contractor options. Earn $34-$36 hourly (W-2) or $38-$40 hourly (1099) with comprehensive benefits and flexible schedules.

Qualifications

• Current NYS OTA License and Registration.
• EMR competence.
• State-required medical documentation for work in a SNF.
• Valid proof of identity and work authorization.
• Malpractice/liability insurance (if applicable).

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting for conditions such as TBI, CVA, Stroke, Parkinson's, MS, Cancer, orthopedic surgery.
• Monitor and document patient progress using EMR software.

Occupational Therapy Assistant Job Duties:

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
