Flagstar Rehab is seeking a Certified Occupational Therapy Assistant (COTA) to support residents in a Skilled Nursing Facility. You will work with a multidisciplinary team to deliver therapy and educate patients and families, following established care plans and using EMR software to track progress.

The position offers flexible scheduling (FT/PT/Per Diem) with competitive hourly pay and benefits for employees, plus potential independent contractor engagement with setup assistance.

Qualifications

• Active NYS OTA license and registration required.
• EMR software experience or competence.
• State required medical documentation for SNF work.
• Proof of identity and work authorization.
• Malpractice/liability insurance if applicable.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Job Duties:

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in a SNF setting, who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.

Occupational Therapy Assistant Requirements:

• Current NYS OTA License and Registration
