Job summary

Flagstar Rehab is seeking Occupational Therapists (OTs) for Skilled Nursing Facilities in New York. This position offers flexible assignments to fit your schedule, with a choose-the-way-you-earn model and optional corporate setup for independent work.

You will provide evaluations and individualized treatment plans, supervise COTAs, and deliver quality therapy to SNF patients with conditions such as stroke, TBI, Parkinson’s, MS, and orthopedic surgery.

Qualifications

• Current NYS OT License and Registration.
• Valid BLS Certification.
• EMR Competence.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to SNF residents with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, Cancer, orthopedic surgery.
• Monitor and document patient progress with EMR software.

Pay Rate

$46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Occupational Therapist Requirements and Responsibilities

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence

• Monitor and document patient progress with the use of innovative EMR software.
