Zusammenfassung

firsthand Health is seeking a Core Guide to support individuals living with serious mental illness. You will leverage lived experience to build trust, schedule appointments, and link clients to behavioral and physical wellness resources.

The Core Guide partners with clinicians and care teams to ensure access to services, assist with transportation, and advocate for client goals in a trauma-informed, whole-health framework. This is a full-time, in-person role in the Tacoma area.

Qualifikationen

• Lived experience with SMI/SUD and recovery
• Peer Recovery certification or eligibility to obtain within 6 months
• Valid driver’s license and willingness to drive
• Proficiency with email, messaging apps and common apps
• Two years recovery time from mental health and/or substance use disorder
• Paid training prior to serving individuals and passing assessments
• Mon–Fri 8:30 am–4:30 pm availability
• Experience with direct in-person outreach and engagement
• Knowledge of recovery education and coaching
• Understanding of whole health and trauma-informed peer support
• Familiarity with local community resources and health systems
• Eagerness to expand knowledge across health conditions

Aufgaben

• Conduct community outreach to individuals with SMI in homes and care settings
• Schedule appointments and assist with telehealth access
• Build trust and empower individuals to pursue personal recovery goals
• Serve as primary contact and facilitate ongoing engagement
• Facilitate access to benefits, resources, and transportation
• Assist with care coordination and post-discharge needs
• Document encounters and participate in team reviews
• Perform initial and ongoing telephonic outreach
• Participate in ongoing training and development
• Role evolves with organization needs
