Job summary

firsthand Health is hiring a Core Guide (Peer Recovery Specialist) to engage individuals with SMI, schedule care, and connect them with supportive services. You will use lived experience to build trust, coordinate care, and help access benefits in a collaborative multidisciplinary team.

The role emphasizes outreach, documentation, and ongoing training, with a Monday–Friday schedule and opportunities for growth within a values-driven organization.

Qualifications

• Lived experience with SMI and/or SUD.
• State Peer Recovery certification, or eligibility and willingness to obtain certification within six months.
• Valid driver’s license in good standing and ability to operate a vehicle.
• Two years recovery time from a mental health and/or substance use disorder.
• Paid training period with assessments and growth.
• Monday–Friday 8:30 am–4:30 pm availability.
• Direct in-person outreach and engagement for individuals with SMI/SUD.
• Interest in gaining CHW credential after hire.

Responsibilities

• Conduct community outreach to individuals with SMI in homes, community locations, and care settings.
• Schedule appointments, take vitals, ensure visits are attended, and assist telehealth access.
• Build trust and relationships to empower individuals to pursue recovery goals.
• Serve as the primary point-of-contact to facilitate ongoing connection and engagement in services.
• Address social determinants of health and facilitate access to benefits and resources.
• Assist with care coordination and accompany individuals to appointments.
• Provide timely documentation of encounters and participate in case reviews.
• Perform initial and ongoing telephonic outreach.
• Engage in ongoing training and professional development.
