Job summary

firsthand is seeking a Core Guide to engage individuals living with SMI, schedule appointments, advocate for needs, and support goals within a multidisciplinary care team. You will build trust through empathetic outreach, coordinate care, document encounters, and participate in ongoing training.

Qualifications

• Lived experience with SMI and/or SUD
• State Peer Recovery certification or eligibility and willingness to obtain within six months
• Valid driver’s license with ability to operate a vehicle (company vehicle may be provided) and able to drive under the company’s insurance policy

Responsibilities

• Conduct community outreach to individuals with SMI in homes, community locations, and care settings
• Schedule appointments and ensure clinical visits are attended; assist with telehealth access
• Build trust by addressing immediate needs and help individuals identify and pursue personal recovery goals
• Serve as primary point of contact to facilitate ongoing connection with supportive services
• Assist with social determinants of health and facilitate access to benefits, resources, and transportation
• Assist with care coordination, accompany to appointments, and address post-discharge needs
• Provide timely documentation of encounters and participate in case reviews and team collaboration
• Perform initial and ongoing telephonic outreach
• Participate in ongoing training and professional development
• Role evolves with organization; duties may change over time
