Job summary

Firsthand is hiring a Core Guide to support individuals living with SMI within the community. The role is rooted in peer recovery, outreach, and ongoing engagement with clients to promote holistic well-being.

You will work with a multidisciplinary team, scheduling visits, conducting outreach, documenting interactions, and helping clients access benefits and services. This is mission-driven, with a focus on trust, growth, and recovery journeys.

Qualifications

• Lived experience with SMI and/or SUD.
• State Peer Recovery certification, or eligibility to obtain within six months of employment.
• Valid driver's license in good standing and ability to operate a vehicle.
• Two years demonstrated recovery time from a mental health and/or substance use disorder.
• Paid training period with assessments before serving individuals.

Responsibilities

• Conduct community outreach to individuals with SMI in homes, community locations, and care settings like shelters, hospitals, and emergency departments.
• Schedule appointments, take vitals, ensure clinical visits are attended, and assist with telehealth access.
• Build trust and relationships by addressing immediate needs and empowering individuals to pursue personal recovery goals.
• Serve as the primary point-of-contact, using advanced engagement techniques to facilitate ongoing connection and engagement in supportive services.
• Help address social determinants of health, facilitating access to benefits, resources, and transportation.
• Assist with care coordination, advocating for individual goals, accompanying to appointments, and addressing post-discharge needs.
• Provide timely documentation of encounters and participate in case reviews and team collaboration.
• Perform initial and ongoing telephonic outreach.
• Participate in ongoing training and professional development.
• Be prepared for role evolution as firsthand learns and grows.
