Job summary

firsthand seeks a Core Guide to support individuals living with SMI and/or SUD, delivering outreach, engagement, and care coordination in collaboration with clinicians and peers. This full-time role emphasizes trust-building, documentation, and ongoing training within a supportive, growth-focused team.

You will schedule appointments, assist with telehealth, and advocate for clients to access needed services, benefiting from a comprehensive benefits package and a mission-driven environment.

Qualifications

  • Lived experience with SMI and/or SUD.
  • State Peer Recovery certification, or eligibility and willingness to obtain certification within six months.
  • Valid driver’s license in good standing and eligible to drive.
  • Proficiency with email, messaging services, and apps.
  • Two years recovery time from a mental health and/or substance abuse disorder.
  • Paid training period with assessments to progress.
  • Monday - Friday 8:30 am - 4:30 pm.

Responsibilities

  • Conduct community outreach to individuals with SMI in homes, community locations, and care settings.
  • Schedule appointments, take vitals, ensure visits are attended, and assist with telehealth access.
  • Build trust and empower individuals to pursue personal recovery goals.
  • Serve as the primary point of contact, facilitating ongoing connection to services.
  • Assist with care coordination, advocating for goals and accompanying to appointments.
  • Provide documentation of encounters and participate in case reviews and team collaboration.
  • Perform ongoing telephonic outreach and participate in professional development.
  • Role evolves with experience and needs of the program.
