firsthand Health seeks a Core Guide (Peer Recovery Specialist) to engage individuals with SMI, schedule care, and support recovery through outreach and coordinated services. This role emphasizes trust-building, trauma-informed peer support, and collaboration with clinicians within a multidisciplinary Care Team.

The position involves direct outreach, appointment advocacy, and documentation, with ongoing training and a strong emphasis on lived experience guiding care.

Qualifications

• Lived experience with SMI and/or SUD.
• State Peer Recovery certification, or eligibility to obtain within six months.
• Valid driver's license in good standing.
• Proficiency with basic digital tools (email, apps).

Responsibilities

• Conduct community outreach to individuals with SMI in homes and community settings.
• Schedule appointments, take vitals, ensure visit attendance, assist telehealth access.
• Build trust and empower clients to pursue personal recovery goals.
• Serve as primary contact and coordinate ongoing engagement in services.
• Address social determinants of health and connect to benefits and transportation.
• Advocate for goals, accompany to appointments, address post-discharge needs.
• Document encounters timely and participate in case reviews.
• Perform ongoing telephonic outreach and training participation.
• Role evolves with ongoing learning and feedback.
