Commonwealth Senior Living in Woodstock Valley, CT is seeking a dedicated Resident Care Associate (RCA) to support residents in Assisted Living and Memory Care. The role emphasizes gentle daily assistance, adherence to individual care plans, accurate documentation, and enhancing residents' dignity and quality of life through empathy and teamwork.

Qualified candidates include CNAs or HHAs or those willing to obtain certification.

Qualifications

• CNA or HHA certification is required.
• CPR and First Aid certification, or willingness to obtain per company policy.
• Ability to fulfill the physical and emotional needs of residents.
• Upholds resident confidentiality.
• Flexible availability, including weekends.

Responsibilities

• Directly assist residents with daily tasks, coordinating with the Resident Care Director.
• Aid residents with mobility using proper techniques and equipment.
• Conduct routine rounds per individualized care plans.
• Assist with lifts, moves, and transport to prevent injury.
• Maintain skin integrity by careful care and incontinence management.
• Document daily care activities per policy and standards.
• Report resident issues promptly to supervisors.
• Respond to inquiries from residents, visitors, and staff per policy.
• Familiarize with individualized care plans and provide support accordingly.
• Contribute observations to care planning with residents' needs and preferences.
• Interact respectfully with residents, families, visitors, peers, and supervisors.
• Provide physical, emotional, and psychosocial support to residents and families.
• Adhere to residents' rights and report any incidents.
• Follow safety and infection control procedures and hand hygiene.
• Attend required training, in-service programs, and staff meetings.
• Maintain regular contact with the Resident Care Director.
• Fulfill other assigned duties.
