One Senior Care is seeking a Participant Enrollment Specialist to guide seniors and families through the enrollment process. You will build relationships, complete assessments, and coordinate documentation, working with our interdisciplinary team to ensure a smooth enrollment experience.

Ideal candidates are compassionate, highly organized, and able to juggle multiple priorities. The role supports a mission-driven team dedicated to helping seniors remain at home and connected to care.

• Excellent communication and relationship-building skills.
• Highly organized with strong follow-through and ability to manage multiple priorities.

Responsabilités

• Meet with potential participants and families to explain the program and assess needs.
• Complete in-home and community-based assessments for prospective participants.
• Coordinate required evaluations, documentation, and intake steps.
• Attend intake appointments and support participants through enrollment.
• Maintain accurate records and updates in the CRM and EMR.
• Collaborate with Medicaid Enrollment Specialist for smooth transitions.
