Qualifications
One year of experience with the frail and elderly population or willingness to train.
Knowledge of Medicare/Medicaid regulations.
Knowledge of sales and marketing strategies.
Responsibilities
Evaluate potential participants to determine eligibility and program fit.
Complete initial assessments, including home-based assessments.
Coordinate enrollment activities with state personnel.
