Job summary

One Senior Care is seeking a Participant Enrollment Specialist to guide seniors and families through joining our programs. You’ll build relationships, complete assessments, coordinate documentation, and support each prospective participant through enrollment, collaborating with families, community partners, and our Interdisciplinary Team.

Responsibilities

• Meet with potential participants and families to explain the program and assess needs.
• Complete in-home and community assessments for prospective Participants.
• Coordinate required evaluations, documentation, and intake steps.
• Attend intake appointments and support Participants through enrollment.
• Maintain accurate records in the CRM and EMR and coordinate with Medicaid enrollment staff.
• Support team meetings, training, and quality improvement activities.
