Responsibilities

• Provide routine daily care per care plans.
• Use PCC to ensure correct resident information for ALDS and meals.
• Report changes to Health and Wellness Director ASAP.
• Greet and assist residents to rooms; maintain a warm environment.
• Record entries accurately on notes and incident reports.
• Maintain confidentiality of resident information (HIPAA).
• Assist with mobility, transfers, and daily activities as trained.
• Observe for skin breakdown and prevent decubitus ulcers.
