Job summary

Primary Health Solutions is seeking an experienced Case Manager to deliver CPST services to adults 18+. You will develop collaborative, goal-oriented treatment plans and support clients in achieving outcomes in community-based settings.

This role reports to the Assistant Managers of Traditional Case Management and collaborates with a multidisciplinary team. The position requires strong time management, independence, and adherence to HIPAA guidelines, with a focus on client-centered care and

Qualifications

• Experience providing case management to adults with mental illness.
• Bachelor's degree or higher in related field; or an associate degree with 3+ years in the field.
• Ability to read, write and communicate clearly; competency in safety rules and procedures.
• Knowledge of HIPAA and maintaining client confidentiality.
• Ability to work independently and within a multidisciplinary team.
• Strong time-management and organizational skills.

Responsibilities

• Coordinate ongoing assessment and monitoring of clients and families to support symptom management and optimal functioning.
• Provide crisis intervention services for clients in acute distress.
• Educate clients and connect them to community resources and services.
• Monitor service delivery and advocate for clients across providers to ensure coordinated care.
• Lead or co-facilitate therapeutic groups aligned with ISP goals.
• Complete documentation and reporting accurately and on time; maintain 90% accuracy in clinical records.
