Job summary

Outagamie County is seeking a Community Mental Health Case Manager for CCS Kids and Family in Appleton, WI. You will manage assigned clients, coordinate services, and document care plans with an interdisciplinary team to meet individualized goals.

The role requires compassionate communication, strong organization, and the ability to balance crisis response with routine case management. You will transport clients to services, maintain records, and support community integration.

Qualifications

• Bachelor's degree in behavioral science or a related field is required.
• Two years of experience with the respective target group preferred.
• Must possess and maintain a valid driver's license.

Responsibilities

• Provides protection and advocacy services including oversight of Chapters 51, 54 and 55.
• Transports and escorts consumers to meetings, services, appointments and other activities.
• Attends Unit and other meetings as required.
• Maintains accurate records and prepares reports and documentation for court proceedings.
• Maintains clinical files or accurate records and prepares reports for coordination of the case and by State and Medical Assistance guidelines.
• Responds to client needs in crisis situations during work and off hours; evaluates risks.
• Provides ongoing symptom assessment and monitors client’s stability and treatment requirements at each contact.
• Manages a caseload of individuals with severe and persistent mental illness focusing on supportive therapy and community integration.
• Provides on-call services as assigned.
• Works with an interdisciplinary team to coordinate services.
