Family & Children's Center is seeking a compassionate Case Manager for our Community Support Program in La Crosse, WI. You will help individuals with severe mental illness navigate challenges, connect with resources, and work toward recovery in clients' homes and communities.

The role emphasizes relationship-centered care, strong advocacy, and collaboration with families and providers. Local travel and community-based settings are essential aspects of the position.

Qualifications

• Bachelor's degree in behavioral science or related field with 1,000 hours of supervised post‑degree clinical experience with persons with chronic mental illness; OR
• Bachelor's degree in another field with 2,000 hours of supervised post‑degree clinical experience with persons with chronic mental illness.
• Familiarity with the human service delivery system, DHS 63, recovery principles, ethics, and professional boundaries.
• Valid driver's license and ability to use own vehicle for client transportation.

Responsibilities

• Provide mental health case management services to individuals with severe and persistent mental illness.
• Complete assessments and create treatment, recovery, and crisis plans.
• Help clients identify and achieve individual recovery goals.
• Provide strengths-based, recovery-oriented services.
• Connect clients with housing, food, healthcare, safety, and independent living resources.
