Job summary

WestCare seeks a Case Manager to provide person-centered case management and care coordination for individuals served by the Wisconsin Mobile Mental Health Unit. You will conduct psychosocial assessments, develop individualized service plans, coordinate referrals, and support engagement in treatment and long‑term stability.

Qualifications

• Minimum one year of experience in case management, behavioral health, crisis intervention, or related human services.
• Experience with mental illness, substance use disorders, or housing instability is preferred.
• Bachelor's degree in Social Work, Psychology, Human Services, Counseling, Sociology or related field is strongly preferred.

Responsibilities

• Engage individuals using trauma-informed, person-centered approach.
• Conduct psychosocial assessments and develop individualized service plans.
• Coordinate referrals and warm handoffs to behavioral health, housing, medical, and social services.
• Transport clients as needed and operate mobile unit safely.
• Maintain documentation per program requirements and communicate changes to the team.
• Participate in interdisciplinary case consultations and quality-improvement activities.
