Qualifications

• Bachelor's degree in Social Work or related field is required.
• Val id driver’s license and ability to pass background checks are required.
• Strong communication, organization, problem-solving and time-management skills.
• 1+ year experience in mental health or related field preferred.
• Master's degree in Social Work or related field preferred.

Responsibilities

• Build trusting, respectful relationships with individuals and families.
• Provide trauma-informed, person-centered, strengths-based care coordination.
• Develop and implement individualized recovery plans with clients and families.
• Connect individuals to mental health, substance use, healthcare, social, and community resources.
• Complete referrals and coordinate services with providers.
