• Criminal and caregiver background checks, driver’s record review, and drug screening are required.
• Valid driver’s license, reliable vehicle, and sufficient insurance to meet agency requirements.

Responsibilities

• Build meaningful relationships by meeting individuals where they are and building trust.
• Coordinate individualized care with families, providers, and recovery teams to develop recovery plans.
• Connect people to resources by identifying community services and completing referrals.
• Collaborate with providers to promote coordinated, high-quality care.
• Support recovery goals by monitoring progress and providing coaching and psychosocial support.
